Report forms for regulatory interviews at 6 months, 18 months, and 30 months (or 9 months and 20 months under the old system) can be found below:

• CR CSI 9M (for 1st registration before 01/09/23)
• CR CSI 20M (for 1st registration before 01/09/23)
• CR CSI 6M (for 1st registration AFTER 01/09/23)
• CR CSI 18M (for 1st registration AFTER 01/09/23)
• CR CSI 30M (for 1st registration AFTER 01/09/23)

These forms simplify the drafting process while ensuring that all important questions are addressed and that the advice provided is clear, particularly regarding the agreement for re-registration.

These reports, completed by the CSI,are those which must be entered into SYGAL, not the progress documents sent by the PhD students to the CSI prior to the interviews.
